The Pitch

Problem statement

Reaching Communities: Families & Community Support Service provides a first-response service for vulnerable adults/ individuals and families wanting to access social care within the provision of the Care Act 2014 in London. This includes advocacy, advice, information, and support. We provide independent and high-quality information, legal advice, and advocacy on matters relating to health, social welfare, and social care to strengthen integration between health, social care, and community-based services, ensuring residents are provided with the right help as quickly as possible. We help vulnerable adults and families who, due to disability are unable to express themselves.

Why are we best placed to deliver?

We have consulted with our local community via both formal and informal methods including individual and group consultations to understand the challenges they are facing and the additional support they require. Our (Empowered Voices) project aims to increase the confidence of individuals and families by empowering and supporting them to achieve and sustain equality and empowerment. We promote Equality, Diversity and Inclusion through Advocacy and Empowerment for and behalf of individuals and families suffering or likely to be at risk of suffering discrimination. We have a highly dedicated and experienced team consisting of Social Workers, Housing Officers, and experience volunteers etc.

Expected impact

If the grant is awarded; it anticipated that over 150 individuals, families and vulnerable adults from including those from Ukraine will benefit from this initiative. • One of the main expected outcomes of the project will be improvement of individuals and families’ who had arrived from Ukraine lives and empowering the entire community and build confidence or self-esteem. • Every little helps – this project will provide support for individuals and families from experiencing poverty and hardship by providing support and help to remove initial financial barriers that are preventing individuals and families from achieving their full potential. • The project/ initiative aims to provide timely, reliable and accurate and useful information to individuals, families and vulnerable adults who and especially families who had arrived from Ukraine requiring advice and advocacy or support for their voice to be heard. • It will promote equality, social justice, social inclusion and human rights
